accurately reflects my experience and qualifications for work. Furthermore, I understand that an offer of employment made to me is based upon the accuracy and validity of the information contained within this application and supporting documentation, and that offer may be withdrawn or the contract terminated if this information is found to be incorrect or misrepresented in anyway.
2. I have disclosed all relevant information in relation to my mental and physical ability to safely carry out the inherent requirements of the position(s) for which I have applied.
3. I understand that any offer of employment may be subject to checks which may include, but are not limited to: former employers,
criminal background check, visa checks and pre-employment medical assessments.
4. I understand that personal information collected via this application form will be used for the purpose of recruitment by IGA Norris Park and that such information may be disclosed to referees and employees of IGA Norris Park who are involved in the recruitment process.
5. I understand that personal information collected via this application form will not be disclosed to any third parties without my prior consent unless required by law to do so.
6. Should I not be offered employment, I understand that the information provided in this application form and in the supporting documentation will be kept on file for up to three (3) months, after which it will be destroyed.
7. I consent to IGA Norris PARK using and disclosing my personal information in the manner described in (4), (5) and (6) above.
